Mantadin 100mg Capsule is a dual-action medication that functions as both a dopamine agonist for treating Parkinson's disease and an antiviral agent against certain viral infections. By elevating dopamine levels in the brain, it enhances motor control and mitigates Parkinson's symptoms. Simultaneously, it inhibits viral growth and reproduction, preventing the release of infectious viral components, thus thwarting influenza infection.

Indication of Mantadin 100mg Capsule

AMANTADINE contains amantadine, a dopamine agonist that serves as an anti-Parkinson's medication. Its mechanism involves elevating dopamine levels in the brain, a crucial chemical responsible for enhancing the coordination of body movements. This effect contributes to improved daily functioning and an enhanced quality of life for individuals with Parkinson's disease. Notably, Parkinson's disease is more prevalent among men.

Pharmacology

Amantadine's exact mechanism for its antiviral effects remains unknown. However, it interferes with the viral M2 protein's transmembrane domain, primarily hindering the release of infectious viral nucleic acid into host cells. Additionally, Amantadine can inhibit viral assembly during virus reproduction under specific conditions. Its impact on the immunogenicity of inactivated influenza A virus vaccines is minimal.

Amantadine effectively prevents replication of influenza A virus isolates across all subtypes, including H1N1, H2N2, and H3N2. However, it demonstrates limited or no activity against anti-influenza B viral isolates. While in vitro studies have shown varying results regarding Amantadine's effectiveness against influenza A, no clear link has been established between in vitro susceptibility and clinical response. The dose required to suppress viral growth by 50% (ED50) varies widely (0.1 g/mL to 25.0 g/mL) based on assay techniques, virus strains, and cell types used.

Dosage & Administration of Mantadin 100mg Capsule

Dosages for various conditions:

Parkinson's disease:

• Adult: Initially, 100 mg/day, increased to 100 mg twice daily after a week or more. Maximum dose: 400 mg daily.
• Elderly (>65 years): Start with the lowest effective dose.

Prophylaxis of Influenza A:

• Adult: Take 100 mg daily for up to 6 weeks; if used with influenza vaccination, take up to 3 weeks after vaccination.
• Child (ages 10-15 years): Take 100 mg daily.

Influenza A:

• Adult: Take 100 mg daily for 5 days.
• Elderly (>65 years): Take a single dose of 100 mg.

Herpes zoster in immunocompromised patients:

• Adult: Take 100 mg twice daily for 14 days, and continue for another 14 days if pain persists.

Note: Due to resistance, amantadine is no longer recommended for influenza prophylaxis or treatment.

• Swallow the tablet whole with enough water.
• Follow your doctor's prescription precisely.
• Do not cut, break, or chew the tablet.
• Do not exceed the prescribed dose.
• Do not stop the medication without consulting your doctor.

Interaction

• Certain medications can interact with Mantadin 100mg Capsules, potentially affecting their efficacy or causing interactions. It's crucial to inform your doctor about all medications, supplements, or herbal products you're currently taking or planning to take to prevent any potential issues.
• Using Mantadin 100mg Capsules alongside medications that stimulate the central nervous system may lead to central nervous system toxicity. Combining this medicine with levodopa or anticholinergic agents like procyclidine (used for Parkinson's disease management) could result in symptoms such as confusion and gastrointestinal disturbances.
• In combination with thioridazine (used for mental disorder management), Mantadin 100mg Capsules may amplify tremors in the elderly with Parkinson's disease.
• Take special caution if you're on medications for high blood pressure/fluid accumulation (diuretics like hydrochlorothiazide, potassium-sparing diuretics), malaria (quinine), or irregular heart rhythm (quinidine).
• Avoid administering live attenuated influenza vaccine (LAIV) within two weeks before or 48 hours after using Mantadin 100mg Capsules.

Contraindications

If you are allergic to amantadine or to any of the other ingredients of Mantadin 100mg Capsules.

Pregnancy & Lactation

Category C pregnancy. There has been no well-controlled research on Mantadin's safety in pregnant women. When the possible advantages outweigh the unknown dangers to the fetus, amantadine may be administered during pregnancy.

Low levels of amantadine are excreted in breast milk. Because there is no evidence on the effects of amantadine on babies, it should be taken with caution in women who are nursing.

Precautions & Warnings

Consult your doctor if:

If combined with anticholinergic drugs, adjust the doses to avoid atropine-like symptoms. Abrupt cessation can cause side effects like delirium, agitation, hallucinations, depression, and speech difficulties. Always consult your doctor before making any changes to your medication.
